SMP for men who keep their hair.
By Steven “Stevey G” Greitzer · September 5, 2026
Scalp micropigmentation does not require a shaved head. For a man with thinning hair who keeps his length, impressions are placed on the scalp between the hairs he still has, so the skin stops showing through and the hair looks dense again at the length he already wears it. The principle is the same as shaved-head work, but we adjust the size, colour and placement of the impressions from the start.
What it does, and what it does not
SMP reduces the contrast that makes thinning visible. Pale scalp shows between darker hairs, most of all under overhead light, in photographs, and in the mirror at the barber, where the light comes from directly above and there is nowhere for the scalp to hide. Put the colour of hair back on the scalp between those hairs and the contrast is gone, and once the contrast is gone the eye sees coverage. That change is visible from arm’s length and from further away.
It doesn’t grow or attach hair, so you’ll still be combing and styling the same hair after treatment. Where hair is thin, it comes across as thick. Where hair is absent across a wide area there is nothing for the impressions to sit among, and the plan there is either a shaved-head result or a conversation about what a shorter cut would allow. We decide that at consultation, from your own head rather than from a rule.
Impressions between existing hair
On a shaved head every impression stands in for a follicle. Under longer hair it sits between real hairs and darkens the skin beneath them. So the impressions are placed finer and the colour is a touch softer, matched to your hair colour and mixed for your skin so it heals as a shadow of your own density rather than as marks. The mix carries a corrective chosen from your undertone, which is what keeps the healed colour neutral over the years, never grey and never blue, against whatever colour your hair is now.
The hair is parted and worked in sections; nothing is cut. You leave with it combed the way you came in. One client last summer went straight from the chair to a dinner, and nobody at the table knew where he had spent the afternoon.
The crown and the part
These are the two places men with length ask about most. The crown thins in a whorl, and the whorl is where scalp shows first, because the hair grows away from a centre point and the light lands straight on the skin. The work there follows the whorl’s direction, so the shadow sits where hair would throw it on its own.
A part is a line of scalp on purpose, and the aim isn’t to erase it but to narrow it. The strip of skin either side goes darker, and the part becomes a line again instead of a stripe. The same goes for a hairline that has crept back under a fringe or a side sweep. The shape is mapped where it thinned and blended into the growth around it, so there is no visible seam between the work and your own hair.
How it is built
The treatment takes a minimum of three sessions, including the final refinement, spaced ten to fourteen days apart. The first session lays the base across the area and shows what your skin keeps. The second is placed over healed work, once the first has settled to its true value, and it’s during that second pass that the density stops looking applied and starts looking like yours. The third refines the balance across the crown, the edge of the part, and any spot that healed lighter. Building gradually is what keeps the result soft under hair. Forcing density in one sitting is how work ends up looking dark and drawn on.
Sessions take a few hours. There’s no downtime that shows. For the first days the scalp is treated gently, with no washing for a short period, no hard sweating and no sun, and after that you return to your usual routine. You wash, you style, you go to the barber.
Maintenance as hair changes
Hair keeps changing after the work is done, and the work is planned for that. If thinning continues, the area is extended in a later session and blended into what’s already there. If you go greyer, the healed colour was neutral to begin with, and it sits under grey hair as a shadow the way it did under brown. If one day you decide to shave it all off, the density work becomes the base for a shaved-head result and is built up from there rather than started again. And if after some years you’d like it to pop again, a single refresh session brings it back to its first value. How long SMP lasts covers the timeline.
Cost
Treatment is from $2,500 with all sessions included. You get the exact figure in writing after a free consultation. What moves the number is the area treated and the density asked for. The Miami cost page and the New York cost page walk through it.
Where to start
Photographs of the crown and the part under overhead light, sent ahead, tell most of the story. The consultation is free and private, online or at the Miami or New York studio, and you leave knowing what your head would look like at your length, how it would be built and what it would cost. Or, if you’d rather see a number before you see anyone, request a quote and we’ll start there.
Do you have to shave your head for SMP?
No. For a man with thinning hair who keeps his length, impressions are placed on the scalp between the hairs he still has, so the skin stops showing through and the hair looks dense again at the length he already wears it. The hair is parted and worked in sections, nothing is cut, and you leave with it combed the way you came in.
Does SMP work on the crown?
Yes. The crown thins in a whorl, and the whorl is where scalp shows first, because the hair grows away from a centre point and the light lands straight on the skin. The work there follows the whorl's direction, so the shadow sits where hair would throw it on its own, and the eye reads coverage.
Will SMP make my hair grow?
No. SMP does not grow or attach hair, so you will still be combing and styling the same hair after treatment. What it does is remove the contrast that makes thinning visible, by putting the colour of hair back on the scalp between the hairs you have. Where hair is thin, it comes across as thick.
What happens to SMP if I keep losing hair or go grey?
The work is planned for hair that keeps changing. If thinning continues, the area is extended in a later session and blended into what is already there. If you go greyer, the healed colour was neutral to begin with and sits under grey hair as a shadow. If you one day shave it all off, the density work becomes the base for a shaved-head result.
How much does SMP cost for thinning hair?
Treatment is from $2,500, all sessions included, with the exact figure given in writing after a free consultation. What moves the number is the area treated and the density asked for. It takes a minimum of three sessions, including the final refinement, spaced ten to fourteen days apart, and sessions take a few hours with no downtime that shows.
